Application for payments:

• When applying for a residence grant, a certificate from the parent’s specialist doctor needs to be submitted with the application. A residence grant certificate shall be submitted using the form available in the health care history system. We are working on connecting the certificate in Heilsuvera.is. 
• The certificate must stipulate the specialist doctor’s reasoning for the necessity of the relevant parent to live far from her home for a certain amount of time before the expected date of delivery relating to necessary maternity services because of the birth of a child, “regarding, for instance, distance, bad weather, strike or pregnancy risk factors”. 
• The certificate must also stipulate information about whether the parent has stayed in a hospital or a health institute during that period. 

• The original certificate must be submitted with a doctor’s signature.
